Below are two Incident Summaries each involving a Caught Between. Please review.

1.     Incident-Injury-Caught Between

A modernization crew parked car 1 on the 9th floor using Independent Service. The mechanic was working in the pit from an 8’ step ladder with the apprentice assisting from the first-floor landing. The apprentice closed the doors to get more wire connectors from upstairs. When the hoistway doors of car 1 closed, the car went into Phase I Fire Service recall and came down on top of the mechanic. His right arm was caught between the car and hoistway sills before he could get off the ladder.



2.
     Incident-Injury-Caught Between

A mechanic was using a rag to clean hoist ropes while he had the controller running the car from top to bottom. The rag became caught in the hoist ropes and pulled his hand into the drive sheave.
