Below are three Incident Summaries involving an Incident-Close Call-Rigging & Hoisting, an Incident-Injury-Fall, and an Incident-Injury-Caught Between. Please review.

Hazard:

1.     Incident-Close Call-Hoisting &Rigging

A mechanic was installing rail brackets and rails from a temporary work platform. The apprentice was using a capstan to hoist rails up to the platform and then wait for the mechanic to finish installing the brackets. There was miscommunication during hoisting, and the apprentice let the rope off the hoist and a rail fell into the pit damaging the counterweight frame. The mechanic and apprentice admitted the radios they were using had static interference.

2.     Incident-Injury-Fall

A mechanic fell exiting a roof top machine room when the wooden stairs collapsed. He injured his elbow and knee.

3.     Incident-Injury-Caught Between

A mechanic was pushing wire through the hoistway wall into the ceiling of a hospital lobby. The adjacent car came down and caught his hand pulling it down into the divider beam causing a compound fracture and torn tendons.
